
An Indiana home care provider says a server breach exposed Social Security numbers and other personal details for 19,851 people.
Advantage Home Health Care says an outsider got into one of its servers on June 9th.
The company learned of the incident on June 16th and confirmed on June 26th that the intruder had taken files that may have included personal information for patients and current or former employees.
The federal breach list puts the count at 19,851.
The company reported the incident on August 25th as a hacking attack on a network server.
For patients, the files could include names, dates of birth, Social Security numbers, addresses, phone numbers and details on medical conditions and care.
For some employees, they could also include job records and health plan information.
The company told federal law enforcement, hired forensic specialists and began mailing letters around August 25th.
It is offering 12 months of credit monitoring, a credit report, a credit score and cyber monitoring through Cyberscout at no charge.
People who got a letter can call 1-888-398-2084.
The company says it is not aware of any fraud tied to the files.
Advantage Home Health Care is an Indiana-owned home care firm with nine locations in 46 counties.
